Job Summary: The Case Manager will provide direct support to participants across multiple areas, including housing assistance, re-entry into society, recovery, food stability, and workforce development. As part of a collaborative team that includes a social worker and a peer, the case manager will assess client needs, coordinate services, and develop personalized action plans aimed at fostering stability and growth for participants. The role requires a balance of advocacy, resource connection, and empathetic support.

Key Responsibilities:

  • Client Assessment & Support:
    • Conduct intake assessments to identify participant needs and goals across housing, re-entry, recovery, food stability, and workforce development.
    • Develop individualized case management plans that outline specific steps and services required to meet client goals.
    • Monitor client progress and adjust action plans as necessary to address changing circumstances.
  • Resource Coordination:
    • Connect participants with appropriate resources, including housing programs, recovery support groups, food security initiatives, and job training opportunities.
    • Collaborate with external agencies, nonprofits, and community organizations to secure the necessary support services.
    • Facilitate access to legal and mental health services as needed, ensuring that all services are culturally and contextually appropriate.
  • Collaboration & Teamwork:
    • Work closely with social workers, peer support specialists, and other team members to ensure a holistic approach to participant care.
    • Participate in regular team meetings to share updates on participant progress and challenges.
    • Engage in case conferences to develop and refine participant strategies as part of a multidisciplinary team.
  • Documentation & Reporting:
    • Maintain accurate and timely case notes, ensuring compliance with organizational and legal requirements.
    • Track participant progress and outcomes, compiling reports for internal review and external funders.
    • Ensure confidentiality and proper handling of sensitive information.
  • Advocacy:
    • Advocate for participant rights and work to eliminate barriers to service access.
    • Provide educational resources and workshops that empower participants to make informed decisions regarding their housing, employment, and recovery.
  • Crisis Intervention:
    • Respond to emergency situations with appropriate crisis intervention strategies, ensuring participant safety and well-being.
    • Work with community partners to coordinate crisis services when necessary.

Qualifications:

  • Education or work experience in Social Work, Human Services, or a related field. 
  • Experience in case management, particularly in housing, re-entry, recovery, food security, or workforce development.
  • Strong understanding of the challenges faced by vulnerable populations, including those in recovery and re-entry processes.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team-based environment.
  • Proficiency in Microsoft Office or Google Workspace and case management software.

Preferred Qualifications:

  • Experience working with diverse populations, including formerly incarcerated individuals and those in recovery.
  • Familiarity with local resources and service networks.
  • Certification in case management or related areas.

Work Environment:

This position may require travel to public or government buildings, community organizations, and partner agencies. Flexibility in scheduling may be required to accommodate the needs of participants.
