Home Health Physical Therapist
We are seeking a qualified Home Health Physical Therapist to provide care and rehabilitation services in a home health setting. The ideal candidate will have a solid background in home care physical therapy with strong knowledge and experience in OASIS and Homecare Homebase.
Key Responsibilities- Conduct thorough assessments of patients, focusing on geriatric and orthopedic specialties.
- Develop and implement personalized home exercise programs for patients.
- Educate patients and their families on therapeutic techniques and adaptive devices.
- Perform documentation in compliance with OASIS standards.
- Utilize equipment such as gait belts and Hoyer lifts to assist patients effectively.
- Address patient needs related to chronic conditions such as COPD and CHF.
- Collaborate with interdisciplinary teams, including social services and interpretation services.
- Minimum of 2 years of experience in home care physical therapy.
- Current DC Physical Therapy license required.
- BLS certification required.
- Experience with Homecare Homebase and OASIS documentation is essential.
- Must have completed at least 1 travel assignment.
- Ability to float to other zip codes as needed based on patient and staffing requirements.
This position involves a day shift, working five days a week from 8:00 AM to 5:00 PM.
Additional Information- Service area covers multiple locations within the region.
- Number of patient visits per day: Approximately 6.
- Mileage reimbursement available for travel between patients.
- Contractors will use their personal vehicles and must ensure they meet all licensure and insurance requirements.
