LTC/SNF LPN

Job Title: Licensed Practical Nurse (LPN)
Setting: Skilled Nursing Facility / Long-Term Care (SNF/LTC)
Employment Type: Full-Time, Part-Time, PRN
Shift: Day Shift Only

Position Summary:
We are seeking a dependable and compassionate Licensed Practical Nurse (LPN) to join our SNF/LTC team on day shift. The LPN will provide direct nursing care to residents while supporting a positive and professional care environment.

Key Responsibilities:

• Provide direct nursing care in accordance with state regulations and facility policies
• Administer medications and treatments as ordered
• Monitor resident conditions and promptly report changes
• Accurately document care provided
• Collaborate with RNs, CNAs, and interdisciplinary team members
• Maintain safety, infection control, and quality care standards

Qualifications:

• Active LPN license in the state of employment
• SNF/LTC experience preferred (new graduates welcome)
• Strong communication and time-management skills
• Compassionate, resident-centered approach
