Speech-Language Pathologist (SLP) – Skilled Nursing Facility (SNF)
\nWe are seeking a skilled and compassionate Speech-Language Pathologist (SLP) to join a collaborative therapy team within a skilled nursing facility. This role focuses on providing high-quality rehabilitation services to short-term rehab patients and long-term residents, helping them improve communication, cognition, and swallowing function.
\nThe SLP will evaluate, treat, and monitor patients with speech, language, cognitive-communication, and dysphagia needs, implementing individualized plans of care under evidence-based guidelines. Strong clinical judgment, communication skills, and teamwork are essential to ensure positive patient outcomes.
\nKey Responsibilities:

Perform comprehensive evaluations for speech, language, cognitive-communication, and swallowing deficits

Develop and implement individualized treatment plans under the supervision of facility guidelines

Provide skilled therapy for residents with neurological, cognitive, or swallowing impairments
\n

Educate patients, families, and caregivers on strategies to improve communication and safe swallowing
\n

Monitor patient progress and communicate updates to the interdisciplinary team
\n

Accurately document evaluations, treatment sessions, and progress notes in EMR
\n

Collaborate with PT, OT, nursing, and case management to ensure coordinated, patient-centered care
\n

Maintain productivity while delivering high-quality rehabilitation services

Ideal Candidate:

Active SLP license in practicing state
\n
- \n

SNF or geriatric rehabilitation experience preferred
\n
- \n
- \n
Strong clinical reasoning and documentation skills
\n

Comfortable managing both short-term rehab and long-term resident caseloads
