We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team, committed to providing exceptional psychosocial support to patients and their families. Duties

  • Conduct comprehensive patient assessments to evaluate emotional, social, and behavioral health needs using clinical counseling techniques and diagnostic tools.
  • Develop tailored care plans that incorporate mental health counseling, behavioral therapy, crisis intervention, and case management strategies.
  • Provide individual and group therapy sessions utilizing modalities such as motivational interviewing, cognitive behavioral therapy, and applied behavior analysis.
  • Collaborate with multidisciplinary teams—including physicians, nurses, therapists, and community agencies—to coordinate patient services effectively.
  • Manage medical documentation in compliance with HIPAA regulations, ensuring accurate records of patient progress and treatment plans using electronic medical record (EMR) systems.
  • Assist patients with intake procedures, discharge planning, and post-acute care coordination for diverse populations including seniors, children, individuals with disabilities or developmental disorders such as autism or Alzheimer’s disease.
  • Support families through child & family counseling sessions and educate them on available resources for hospice care, addiction counseling, or mental health support.
  • Engage in clinical research activities when applicable to improve social work practices and patient outcomes within the healthcare setting.
Requirements:


  • Proven experience in hospital or healthcare environments with familiarity in inpatient or outpatient settings.
  • Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
  • Expertise in behavioral health interventions including crisis management, PTSD care, dementia care, and working with individuals with developmental disabilities or autism spectrum disorder.
  • Proficiency in data collection for research purposes and quality improvement initiatives.
  • Skilled in various therapeutic techniques such as behavioral therapy, psychotherapy, cognitive behavioral therapy (CBT), motivational interviewing, and assisted living support strategies.
  • Experience working with diverse populations including children, geriatrics (geriatrics), hospice patients, or those requiring post-acute or home & community care services.
  • Familiarity with EMR systems for documentation and case management purposes while maintaining strict adherence to HIPAA standards for patient privacy.
  • Ability to perform comprehensive patient assessments involving intake procedures and diagnostic evaluations for mental health or behavioral concerns.
  • Strong communication skills for effective collaboration within multidisciplinary teams and engaging patients/families during sensitive situations.
