Job Title: CWHH-Physical Therapist
Location: Delco, NC
Setting: Home Health
Pay Range: 2530.00 - 2656.50
Schedule: Discuss with recruiter.
Contract Length: 13 weeks.
Candidate Type: Traveler
Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance.
Responsibilities:

Perform clinical assessment to identify patient needs and determine appropriateness of the patient.

Document, prepare/review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis. Revise the POC based on evaluation and ongoing assessment data. Work in consultation with the physician and as part of an interdisciplinary team.

Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments.

Assist/instruct patient/family in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchair, canes, braces, prosthetic devices, and appliances in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs. Order appropriate rehabilitative equipment to meet patients' needs and adapt equipment, appliances to meet home limitations/requirements.

Provide appropriate patient/family education regarding therapeutic procedures based on the patient's age, condition, culture, and learning readiness.

Perform and oversee other team members, when applicable, in the performance of patient care that is consistent with patient's needs and desires.

Apply concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.

Accept clinical assignments that are consistent with education and competence to care for patients.

Perform other related duties as assigned or requested.
