Key Responsibilities:
-
Provide daily personal care (bathing, grooming, dressing, oral hygiene, etc.)
-
Assist with daily living activities based on the Plan of Care
-
Observe and report patient conditions
-
Support with meal prep, feeding, and light housekeeping
-
Accompany patients to medical appointments and essential errands
Requirements:
-
Valid New York State HHA Certificate
-
Government-issued photo ID & Social Security Card (or Work Authorization, if applicable)
-
Two reference letters
-
Recent physical exam
-
TB screening (Quantiferon preferred)
-
Proof of flu vaccination
-
Rubella & Rubeola titers
-
Chain of Custody drug screening (within the last 6 months)
