Responsibilities:

• Provide emotional, spiritual, and practical support to patients and their families, addressing their social, emotional, and spiritual needs

• Conduct patient assessments, develop care plans, and collaborate with interdisciplinary teams to ensure comprehensive care

• Facilitate communication and coordination with healthcare providers, patients, and families to ensure a seamless care experience

• Identify and address social determinants of health, such as access to food, shelter, and healthcare, to promote overall well-being

• Provide bereavement support to families and caregivers, helping them navigate the grieving process

Requirements:

• Master's degree in Social Work (MSW) or related field

• 0 years of experience preferred, but not required

Work Schedule:

• Per Diem position with variable hours, including weekends and holidays

Compensation:

• $45 hourly
