Residential Care Coordinator – Assisted Living Unit (RN)

Reports To:

Director of Nursing / Administrator

Position Summary

The Residential Care Coordinator (RCC) is a Registered Nurse responsible for overseeing the clinical and operational functions of the Assisted Living Unit. The RCC ensures residents

receive safe, high-quality, person-centered care while maintaining compliance with state and federal regulations. This role serves as a leader, mentor, and primary liaison between residents, families, staff, and interdisciplinary team members.

Essential Duties and Responsibilities

Clinical Oversight:

- Conduct nursing assessments, care planning, and ongoing evaluations of residents. - Monitor residents for changes in condition and coordinate appropriate interventions. - Ensure medication administration and treatments are provided per physician orders and regulatory standards. - Oversee infection control and resident safety practices.

Resident & Family Support:

- Maintain positive, professional relationships with residents and families. - Provide education to residents and families regarding health conditions, medications, and care plans. - Serve as primary point of contact for concerns, care updates, and service coordination.

Staff Leadership & Supervision:

- Supervise, coach, and support nursing staff, CNAs, and residential aides. - Develop staff schedules and assignments to ensure adequate coverage. - Provide ongoing training, mentorship, and performance evaluations. - Promote teamwork and foster a culture of respect and accountability.

Regulatory & Quality Compliance:

- Ensure compliance with state assisted living regulations, licensing standards, and facility policies. - Participate in quality improvement initiatives, audits, and surveys. - Maintain accurate documentation in resident records and ensure confidentiality.

Administrative Functions:

- Assist in budget oversight for the ALF unit. - Coordinate admissions, transfers, and discharges in collaboration with the admissions team. - Participate in interdisciplinary team meetings and care conferences. - Prepare reports for leadership regarding clinical outcomes, staffing, and unit needs.

Qualifications

- Current, unencumbered RN license in the state of Maine. - Minimum of 2 years nursing experience; long-term care or assisted living preferred. - Supervisory or leadership experience strongly preferred. - Knowledge of state ALF regulations and best practices in geriatric care. - Strong leadership, organizational, and communication skills. - Ability to manage multiple priorities in a fast-paced environment.
