What You’ll Do
As part of the Air Force Medical Home (AFMH) model, you’ll play a key role in optimizing whole-person care by:

Delivering brief, solution-focused interventions (15–30 mins) for cognitive and behavioral concerns in a primary care setting.

Partnering with Primary Care Managers (PCMs) to identify, assess, and address behavioral health needs.
-
Providing patient education, care planning, and health promotion to support long-term wellness.
-
Leading or participating in group classes on stress management, chronic pain, or sleep hygiene.

Engaging in case management meetings, coordinating referrals, and mentoring clinic staff.
-
Maintaining timely, accurate EMR documentation aligned with MTF and Air Force standards.

Completing TBHC certification training within 90 days of onboarding.
Who We’re Looking For
Minimum Qualifications:
-
Master’s Degree in Social Work (MSW) from a CSWE-accredited program.
-
Active, unrestricted LCSW license in any U.S. state, D.C., Puerto Rico, or U.S. Virgin Islands.
-
At least 24 months (2 years) of clinical experience within the last 36 months, including individual, evidence-based counseling.

Experience using EMRs and working in digital documentation systems.

Trauma-focused experience: Must have been providing CPT, PE, or EMDR regularly (weekly) in the past two years.

Specialized trauma training (EMDR, TF-CBT, CPT, PE, etc.)

Experience in trauma settings (ER, military, corrections, DV, pediatrics, disaster response)

Adult mental health experience: Must have experience with mental health assessment, diagnostics, and treatment planning for adults. Group therapy experience is acceptable but not required.
Preferred Experience:

Prior work in integrated trauma health or primary care within the last 3 years.
