Licensed Practical Nurse LPN

What You’ll Do:
‍ Provide direct nursing care on your assigned shift—medications, treatments, vitals, and procedures
Collaborate with the interdisciplinary team to meet resident care needs
Maintain accurate documentation for assessments, reports, and care plans
Deliver hands-on, compassionate care to support resident well-being
Ensure smooth shift transitions and communicate effectively with incoming/outgoing teams

What You’ll Need:
Current LPN license in New York
Prior LPN experience in a long-term care or skilled nursing setting preferred
Strong assessment, communication, and resident care skills
A team-player mindset with a passion for elder care
