Job Summary:

Rising Stars is seeking a dedicated and compassionate Service Coordinator Early Intervention to join our team in Spring Valley, NY and Nassau County, Ny. As a Service Coordinator, you will play a vital role in supporting early intervention services for children and their families. Key Responsibilities:

• Coordinate early intervention services for children from birth to three years old, in accordance with the New York State Department of Health's Early Intervention Program

• Collaborate with interdisciplinary teams, including therapists, caregivers, and families to develop individualized service plans

• Conduct home visits to assess and evaluate the child's developmental needs and progress

• Communicate with families and healthcare professionals to ensure seamless coordination of services

• Maintain accurate records and reports on client progress and service delivery

• Identify and address any barriers to service delivery, such as accessibility or language issues

Requirements:

• Bachelor's degree in a related field (e.g., special education, social work, early childhood education)

• At least 1 year of experience working with young children and their families, preferably in an early intervention setting

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Familiarity with early intervention policies and procedures

• Valid driver's license and reliable transportation

Compensation:

• Competitive hourly rate of $30 per hour

Work Schedule:

• Part-time schedule, with flexibility to work varying hours, including evenings and weekends
