\n 🩺 **Specialty:** Hospice Registered Nurse (RN)
\n 📅 **Start Date:** ASAP
\n ⏳ **Contract Length:** 13 Weeks
\n 🕒 **Shift:** Days
\n ✅ **Guaranteed Hours:** 32 Hours/Week
\n 💻 **EMR:** Meditech (HospiceSoft/Careficient experience preferred)

\n ### Requirements

\n ✔ Active Utah RN License or Compact RN License
\n ✔ Minimum 3 Years of RN Experience
\n ✔ Hospice, Home Health, or Palliative Care Experience Preferred
\n ✔ Current BLS Certification
\n ✔ Valid Driver's License & Reliable Transportation
\n ✔ Strong Assessment, Communication, and Care Coordination Skills

\n ### Job Responsibilities

\n • Perform comprehensive patient assessments and develop individualized care plans
\n • Provide pain and symptom management for hospice patients
\n • Educate and support patients and families regarding end-of-life care
\n • Collaborate with physicians, social workers, chaplains, and interdisciplinary teams
\n • Maintain accurate and timely documentation
\n • Participate in on-call rotation and weekend coverage as needed

\n ### Additional Details

\n 📞 On-Call: 1–3 shifts per week, including approximately 3 weekends during the contract
\n 🎄 Holiday Coverage Required
\n ⏱ Must be able to respond within 45 minutes when on call
