Exciting Opportunity for a Case Manager - Elevate Your Career!\n\nJob Description:\n- Position: Registered Nurse - Case Manager\n- Shift: Day 5x8-Hour (08:00 - 16:30)\n- Experience Required: Minimum of 2 years in Acute Care Case Management/Discharge Planning\n- Patient Ratio Experience: \n - Days: 1:25 \n - Weekends: 1:45\n- Charting System: Proficiency in Epic required\n- Travel Requirement: Yes (Seasoned traveler preferred)\n\nKey Responsibilities:\n- Conduct assessments and evaluations of patients \n- Coordinate care and facilitate discharge planning\n- Engage in utilization review processes\n- Develop and implement care plans collaboratively\n- Organize and participate in family meetings\n- Ensure compliance with CMS and Joint Commission standards\n- Manage prior authorizations and DME arrangements\n\nPatient Care Details:\n- Patient Age Groups: \n - Neonates \n - Adolescents \n - Adults \n - Geriatrics\n- Daily Census: Manage a case load of up to 25 patients\n- Interdisciplinary Support: Collaborate with Physical Therapy and Social Services\n- Patient Diagnoses: Various medical conditions requiring acute care management\n\nQualifications:\n- Must possess a valid CA state nursing license (Pending license accepted but must be licensed upon start)\n- Strong knowledge in behavioral health is not applicable\n- Ability to handle complex cases involving diverse needs such as homelessness, mental health history, and substance abuse\n- Competence in arranging DME, SNF, and ambulance transport independently\n\nAdditional Information:\n- Orientation Hours: 40\n- Floating Expectation: No\n- Pre-Approved Time Off: 1 Request allowed\n- Weekend Requirement: Every other weekend to meet department needs\n- RTO Restrictions: No more than 7 days\n\nWhy Catalytic Solutions (CatSol): \nAt CatSol, we connect dedicated therapists with rewarding assignments across the country.
