\n Ready for a rewarding assignment where you can make a meaningful difference in patients’ lives from the comfort of their homes? We are seeking an experienced Home Health RN for a 13-week contract in beautiful St. Albans, Vermont.
\n ✨ Assignment Highlights
\n • 13-week contract with an ASAP start
\n • Tuesday–Saturday schedule, 8:00 AM–4:00 PM
\n • On-call rotation with hospice coverage required
\n • Holiday coverage required
\n • McKesson EMR
\n • Vermont RN license required
\n • Minimum 3 years of Home Health RN experience required
\n 🩺 What You’ll Do
\n • Manage a home health caseload and serve as a patient care case manager
\n • Complete OASIS documentation accurately and on time
\n • Conduct patient and caregiver assessments and create individualized plans of care
\n • Provide skilled nursing services, including IV therapy, wound VAC care, complex wound care, medication management, and infection prevention
\n • Collaborate with physicians, LPNs, therapists, social workers, and the interdisciplinary care team
\n • Educate patients and families on treatments, medications, discharge planning, and available support services
\n • Coordinate referrals, transfers, discharges, and home care aide services
