About the Role

Join a growing healthcare organization dedicated to transforming how seniors receive care. As a Medical Assistant, you’ll play an important role in coordinating patient services, supporting providers, and ensuring every patient receives compassionate, high-quality care.

Key Responsibilities

• Support physicians, nurse practitioners, and care teams in daily patient coordination.

• Conduct patient assessments and assist in developing care plans.

• Manage referrals, follow-ups, and communication between patients, providers, and families.

• Handle inbound and outbound patient calls with professionalism and empathy.

• Maintain accurate documentation and electronic health records (EHR).

• Collaborate with healthcare teams to improve workflow and patient outcomes.

• Provide education and guidance to patients and caregivers.

• Perform other duties as needed to support patient care and office operations.

Qualifications

• Medical Assistant Diploma/Certificate required

• Previous experience in senior care, home health, or primary care preferred

• Excellent communication and organizational skills

• Proficient in electronic medical records and scheduling systems

• Strong attention to detail and a commitment to patient satisfaction
