Description
Purpose:
The Start-Up Broker will assist the individual with developmental disabilities to develop a complete Self Direction plan and budget. The Start-Up Broker will assist in developing a Circle of Support, facilitate Person Centered Planning, create an Individualized Budget and write the Self Direction Plan and Budget. The Start-Up Broker will collaborate with the Fiscal Intermediary agency to launch the person’s Self Direction Plan as well as transition it to a Support Broker. The Broker will possess a professional, patient (tolerant) understanding manner and work to maintain a calm, kind, caring and safe environment for the individuals we serve in their home and the community.
The Support Broker will assist individuals with developmental disabilities to Self-Direct Services and Supports through Self Direction. The Broker will provide a variety of ongoing supports to ensure successful implementation, review and updating of the Self Direction plan/budget and Medicaid compliance supporting service billing. The Broker will work with the individual and the Circle of Support to assist the individual in reaching his/her own maximum level of independence and power of potential by promoting a sense of self-worth, acceptance, achievement, citizenship, personal growth and personal choice. The Broker will implement each individual’s person centered plan to assist each individual with improving their skills to be as independent as possible.
Essential Functions:
- To implement, and document each individual’s total person centered plan, encouraging maximum independence and self-worth.
- To monitor/maintain/amend the individuals Self Direction plan to best suit the individual’s goals and maximize their potential. To bill for a pre determined number for Broker Services.
- To assist the family/individual with recruiting staff to teach the individual the person centered goals developed for the Self Direction plan.
- To teach the individual the day to day operation of the residence including meal preparation, inventory, laundry, housekeeping duties, infection control, and purchase of supplies, etc.
- To comply with all state, federal and agency policies, procedures, regulations as indicated by OPWDD.
- To maintain all Individual’s records, ensuring filing, accuracy, compliance, updating.
- To operate agency vehicles in compliance with agency policies/procedures, and traffic laws, ensuring/documenting vehicle maintenance/condition.
- To assist in the coordination/participation/communication and documentation of individual meetings.
- To maintain a calm, tolerant, kind, courteous, accepting environment in compliance with the individuals’ service plan and plan of protective oversight, while in the individuals’ residence and community.
- To provide, ensure a safe work/living environment including compliance with all safety policies/procedures and regulations.
- To attend any trainings, meetings, updates medical requirements necessary/required. To acquire and maintain all required certifications in accordance with agency policy. Attend and participate in staff meetings on a regular basis and assure attendance at all mandatory meetings.
- To foster community inclusion by involving the individual(s) in the planning and implementation of recreational and leisure activities.
- Remain aware/respectful of the individuals’ rights and responsibilities; immediately reporting any concerns or issues of potential harm, mistreatment or rights restriction.
- Embraces an approach of self-determination. Works for and with the individuals on their own individualized outcomes.
Other Duties:
- Participate as an active member of the individuals’ program team and Circle of Support.
- Flexibility of schedule to ensure individual care and program implementation.
- Any and all other related duties as requested by supervisor.
Requirements
Qualifications:
- Must contain Office for People with Developmental Disability (OPWDD) Start Up/ Support Broker Certification which includes: completion of Circle of Support, Self-Advocacy/ Self Determination, Person Center Planning, Broker Training Institute and Self Direction Budget Template Training Completed
- Associate’s Degree in a Human Services Related Field with 2 years experience working with individuals with disabilities; OR Bachelor’s Degree in a Human Services Related Field with 1 year experience working with individuals with disabilities
- CPR/First Aid Certification (training provided)
- Program specific training (provided)
- Valid NYS Driver’s License acceptable to Agency Insurance standards. Maintain valid insurance and provide insurance card if using your own vehicle.
- Ability to exhibit/maintain a professional, kind, courteous and patient manner at all times
Physical / Cognitive Requirements:
- Lifting/transferring – individuals/items of various weights – must be able to lift a minimum of 50 pounds.
- Bending/Reaching/Pushing/Pulling
- Effective verbal and written communication skills
- Reading / Math skills at a level to perform job duties (read-implement physician orders, give medications as ordered, prepare recipes, read program documentation, etc.)
- Demonstrated interest and ability to make possible the personal outcomes of individuals receiving services
- Adherence to Agency policies including attendance policy
