Summary

The Time-Limited Subsidy (TLS) Case Manager provides intensive, short-term case management services to general families and Transition-Age Youth (TAY) families receiving temporary housing or rental subsidies. The position focuses on housing stabilization, income development, and supportive service coordination to support families in achieving long-term housing stability and self-sufficiency by the end of the subsidy period.



ESSENTIAL FUNCTIONS

  • Conduct comprehensive intake and ongoing assessments for general families and TAY families, including housing history, income, education, employment, and supportive service needs
  • Develop individualized housing stabilization and self-sufficiency plans with clear, time-bound goals
  • Provide intensive case management throughout the subsidy period, with frequency based on family needs and program requirements
  • Support TAY parents in accessing education, vocational training, employment, and childcare resources
  • Assist families in increasing income and benefits (employment, CalWORKs/TANF, SSI, SNAP, WIC, childcare subsidies, etc.)
  • Provide budgeting education, tenant rights education, and lease compliance support
  • Coordinate referrals to community-based services, including mental health, substance use, parenting supports, domestic violence resources, and healthcare
  • Collaborate with landlords and housing providers to support housing stability and prevent evictions
  • Monitor subsidy timelines and ensure compliance with program policies and funding requirements
  • Prepare families for subsidy exit by developing transition plans for permanent or sustainable housing
  • Maintain accurate case notes, service plans, and required documentation in the case management system



QUALIFICATIONS

  • Bachelors degree in Social Work, Human Services, Child & Family Studies, or a related field (or equivalent experience)
  • Experience working with families, young parents, or Transition-Age Youth (ages 1824) preferred
  • Knowledge of housing programs, rental assistance, and family-focused service systems
  • Familiarity with trauma-informed, youth-centered, and family-centered practices
  • Strong organizational, communication, and documentation skills
  • Ability to manage multiple families with strict timelines and outcomes
