Job Description
Beloit Health System is looking to add a Physical Therapist to our Team!
- Sign-On Bonus: Yes
- Shift: 1st
- Hours per week: 40

- Department: Lee Lane Physical Medicine
The Physical Therapist evaluates patients and develops goals and a plan of care within the scopе of services. The Physical Therapist reviews the plan and goals and progresses and/or revises the program appropriately.
- Responds to referral and schedules patient according to department policies and procedures.
- Evaluates patient and develops goals and appropriate treatment plan.
- Implements and coordinates Physical Therapy treatment within the overall treatment plan, coordinating care with other professionals as needed.
- Reviews plan of care and progresses or adjusts program as indicated and according to department policy.
- Informs, counsels and consults with patient's family, case managers, industries, etc. and other health care workers.
- Documents patient data (evaluation, treatment and progress notes, D/C summary, charges) and department statistics according to department policies and procedures.
- Provides/recommends adaptive equipment or assistive devices as indicated to increase the independence of the patient.
- Plans discharge with treatment team and provides home program if necessary.
- Supervises and directs physical therapy assistants according to rules/regulations.
- Educates patients and families in regard to functional outcomes and abilities of patient.
- Participates in team conferences and discharge planning with departmental staff and/or members of the interdisciplinary team.
- Provides supervision of Physical Therapy Assistants according to rules/regulations.
- Participates in performance improvements, community and staff education programs.
- Shares in supervision of student interns of Physical Therapy and/or Physical Therapy Assistant programs, or shadow students.
- Performs the necessary functions to prepare, maintain, and clean up the work area to ensure cleanliness and safety.
- The PT has regular access to and is expected to maintain the protected health information of patients in their care.
- Documentation is required and is in accordance with professional, regulatory, and institutional guidelines.
- Strong communication skills and a desire to strive for excellence are essential components of this position.
- Respectfully interacts with a wide variety of internal and external constituents, including (but are not limited to) team members, staff and physicians, local companies, patients, families, vendors, and all insurance representatives.
- Provide coverage to other Physical Medicine locations and settings as necessary based on patient care/departmental needs.
- Work schedules are determined to meet patient care/department needs and shifts may be required on weekends and holidays.
Job Requirements
- Bachelor of Science, Master's Degree, or Doctorate of Physical Therapy from an accredited school. Or higher degree equivalent.
- American Heart Association, Healthcare Provider CPR and AED Certification within 3 months of hire.
- Organization, time management, flexibility, and positive attitude are crucial to successful performance.
- Continuous professional development is expected
Current License in the State of Wisconsin as Physical Therapist at the time of hire or prior to providing services. - Successful completion of pre-placement physical examination and drug screen.
- Ability to read, write, and communicate effectively.
- Ability to recall short- and long-term memory.
- Ability to lift/carry medium work level up to 55lbs.
- Ability to push/pull medium to heavy work level up to 200lbs.
- Ability to complete fine and gross motor movements and eye/hand coordination.
- The physical ability to complete job tasks with full range of body motion necessary to complete dynamic balance, squat, bend, stoop, crawl, reach and sit/stand/walk for extended periods of time.
- Requires ability to see and hear at a level sufficient to perform job duties with or without reasonable accommodation.
- Ability to handle pressures in meeting deadlines and effectively manage multiple tasks throughout the workday.

- Ability to work effectively in a congested work site.
- Ability to work independently as well as in a group setting.
