What You’ll Do:

As a Health Services Coordinator (LPN or RN), you’ll play a hands-on role in improving the lives of our residents by:

  • Conducting comprehensive resident assessments prior to move-in, re-entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.
  • Assisting with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.
  • Establishing and fostering positive relationships with residents, families, associates, home health, hospice, and other external partners.
  • Coordinating and ensuring proper health-related training and education for the caregiver team, conducting skill observations to optimize resident care delivery.
  • Providing direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.

What We’re Looking For:

  • Availability to work Saturdays and Sundays from 8:00 AM – 5:00 PM, with additional on-call availability throughout the weekend.
  • Must hold active state-required nursing licensure (RN or LPN) and an acceptable record. 
  • A current CPR/BLS certification.  
  • 1+ years of experience in a senior living or related industry supervisory or leadership role. 
  • 1+ years of experience providing care to cognitive and memory-care residents or patients (preferred). 
  • Valid driver’s license and acceptable driving record; ability to provide resident transportation and/or serve alcoholic beverages at community events. 
  • The ability to assist residents with limited mobility, lift/push/pull up to 50lbs. 
  • Excellent verbal and written communication skills, with the ability to effectively collaborate with diverse groups, navigate challenging situations, and make sound decisions. 
  • Strong technical skills (e.g., Word, Excel, PowerPoint) and the ability to maintain confidential and privileged information (including HIPAA, residents, and associates).
