Position Overview
The Nurse Assessor at Care Indeed is a critical clinical leadership role responsible for the first impression and ongoing clinical integrity of our client care. This professional bridges the gap between nursing expertise and client relationship management by conducting comprehensive health evaluations and developing individualized care plans. The ideal candidate thrives in a fast-paced environment, completing 3–5 assessments daily through a combination of in-person field visits and virtual telehealth sessions.
Essential Duties And Responsibilities
High-Volume Clinical Evaluations: Conduct a minimum of 3–5 comprehensive assessments per day. This includes initial admissions, quarterly reassessments, and post-hospitalization evaluations.
Hybrid Delivery: Seamlessly transition between in-home assessments from San Francisco -San Jose (requiring local travel) and virtual telehealth evaluations using secure digital platforms.
Assessment Scope: Evaluate clients for physical health status, Activities of Daily Living (ADLs), cognitive impairment, and home safety risks.
Care Plan Development: Design and update data-driven care plans that align with client goals, physician orders, and agency protocols.
Clinical Success Coordination: Act as the primary clinical point of contact for families, providing education and setting expectations for non-medical home care services.
Compliance & Documentation: Ensure all documentation is completed with 100% accuracy and submitted within 4 hours of assessment.
Supervisory Oversight: Conduct supervisory visits to monitor caregiver performance and ensure care plan fidelity.
Job Conditions

The position is stressful in terms of meeting deadlines.
Work may involve virtual or in-person interactions with clients, families, and service providers.
Regular travel is required for client meetings, facility visits, industry seminars, and conferences.
Qualifications
Required
Licensure: Active and unrestricted RN/LVN license in CA.
Experience: Minimum 1–3 years of clinical nursing experience; home health, hospice, or case management experience preferred.
Tech Proficiency: Advanced skills in EHR and telehealth platforms (e.g., Zoom, Microsoft Teams, Google Meet).
Mobility: Valid driver’s license, reliable transportation, and willingness to travel to client homes for in-person evaluations.
Communication: Exceptional interpersonal and clinical writing skills to clearly translate medical needs to non-medical staff and families.
Preferred
Certification: Certified Case Manager (CCM) or Certified Professional in Healthcare Quality (CPHQ).
Multilingual: Proficiency in a second language is a significant plus for diverse client populations.
Physical Requirements
Lifting requirements: Minimal; up to 25 pounds
Regular use of standard office equipment such as laptops, smartphones, and photocopiers.
Work Hours
Monday to Friday, 9:00 AM to 5:30 PM; days may be flexible based on the need. Some weekend and evening work is required in this position.
