Essential Functions:

  • Provide direct patient care under the supervision of registered nurses or physicians.
  • Observe and report changes in patient condition to the supervising RN.
  • Administer medications and treatments according to supervisory orders.
  • Monitor patients' vital signs and assess their condition.
  • Assist in the development and implementation of patient care plans.
  • Document patient information, treatments, and responses to care accurately.
  • Collaborate with other healthcare professionals to ensure coordinated and comprehensive patient care.
  • Maintain a clean and safe patient environment.
  • Adhere to established policies, procedures, and protocols for nursing care.
  • Respond promptly to emergencies and provide appropriate interventions.


Requirements:

  • Valid New Jersey LPN license.
  • Minimum of 2 years of experience.
  • Strong clinical skills and knowledge of geriatric nursing particularly in sub-acute care and assisted living.
  • Excellent communication, interpersonal, and leadership skills.
  • Ability to work collaboratively in a team-oriented environment.
  • Understanding of and commitment to compliance with regulatory requirements.
  • Knowledge of nursing practices, medical terminology, and patient care standards.
  • Ability to multitask and work effectively in a fast-paced environment.
  • CPR and BLS certifications.
  • Flu vaccination required.


Physical Requirements

  • Ability to stand, walk, lift, and move patients for extended periods.
  • Manual dexterity to operate medical equipment.

Work Environment

  • Healthcare setting, long-term care, memory care, nursing home, with potential exposure to infectious diseases.
  • Requires adherence to safety protocols and use of personal protective equipment (PPE).
  • Ability to work shifts, including nights, weekends, and holidays as required.

Per Diem team members are required to work at least two (2) shifts a month and could be asked to work more if there is a company need.
