Job purpose

The Transitional Care Coordinator is responsible for educating patients about our Home Visit program and scheduling appointments to avoid re-hospitalizations. This position cold-calls patients, therefore having a sales-mindset is ideal! This is an in-person position, located at our M&H Housecalls office in Cape Coral FL.

Duties and responsibilities

  • Outbound calls to patients to introduce our Home Visit Program
  • Scheduling patients by geographical area
  • Using proprietary computer database to enter and assign patients to providers
  • Submitting paperwork to Primary Care Physicians
  • Making follow-up calls to patients post-appointment(s) to identify further needs
  • Communication to referral sources in the community

Qualifications

  • Minimum 1 year of experience scheduling patient appointments
  • Experience with sales a plus!
  • Minimum 1 year of experience in a healthcare setting
  • High School Diploma or GED
  • Self-motivated in remote setting; ability to be productive, free of distractions, in a 'home-office' environment (orientations, trainings, meetings may require travel from time to time)
  • Utilization of many computer software systems, our platform, hospital systems, Microsoft etc.
  • Ability to multi-task
  • Bi-Lingual (English & Spanish) preferred not required
  • Excellent written communication skills
  • Must be comfortable being on the phone for several hours a day
  • Must be friendly & courteous and have patience on the phone, as we are very often assisting an elderly demographic
  • Compensation negotiable based on experience
