COMPENSATION & BONUS STRUCTURE

$250,000 Base Salary – New Residents
$295,000 Base Salary – Experienced Physicians
Annual Incentive Bonuses: $45K–$85K+
Bonus potential up to 28% of annual base salary
Potential Total Compensation: $350K–$380K+ annually

PRIMARY RESPONSIBILITIES
━━━━━━━━━━━━━━━━━━━

Evaluate and treat patients according to evidence-based clinical standards
Coordinate specialty referrals, hospital care, SNF coordination, home healthcare, and durable medical equipment needs
Collaborate with the care team during daily huddles and patient care planning
Complete timely and accurate EMR documentation
Participate in quality improvement initiatives and outcome reviews
Deliver patient-centered care in a value-based setting
Support a professional and collaborative clinical environment

REQUIRED QUALIFICATIONS
━━━━━━━━━━━━━━━━━━━

✔️ MD or DO from an accredited medical school
✔️ Active and unrestricted medical license (or willingness to obtain)
✔️ Board Certified or Board Eligible in:
  • Family Medicine
  • Internal Medicine
  • Geriatric Medicine

✔️ Active DEA License
✔️ Medicare & Medicaid Provider eligibility
✔️ Excellent communication and interpersonal skills
✔️ Comfortable working in a collaborative care environment
✔️ Basic EMR/EHR and computer proficiency

✅ Medicare Advantage experience
✅ Knowledge of HEDIS/STARS metrics

✅ Prior value-based care experience preferred
✅ Electronic Medical Record (EMR/EHR) experience
