PACE Place provides coordinated medical and social services to elders who want to remain in their home but need support to do so. PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care.

 

Position Summary

The PACE Transportation Coordinator is responsible for PACE Partners Transportation program, including transportation of participants, meals, durable medical equipment, and medications; transportation scheduling; oversight, hiring, training and supervision of all drivers and dispatchers and any other transportation as deemed necessary.  Oversees vehicle repairs and maintenance.  Identifies quality and cost-effective vendor for same.  Responsible for meeting departmental budget goals.

 

Essential Functions

  • Supervise the transportation departmental staff. 
  • Responsible for departmental hiring, orientation, competency, and training procedures
  • Participate in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of care delivery
  • Communicate interdisciplinary team decisions, participant changes, care plan changes, and updated information to transportation staff on a daily basis
  • Ensure responsibility for running and maintaining transportation scheduling software program
  • Ensure effective daily communication between PACE Partners staff, transportation drivers, and dispatch
  • Ensure the scheduling, coordination, communication of the driver route assignments of PACE Partners and assure the daily work schedule is communicated
  • Ensure the scheduling of transportation for all participants outside appointments
  • Monitor use of driver/dispatch overtime, ensure adequate staffing, and improve efficiencies
  • Coordinate and schedule the daily delivery of home delivered meals, durable medical equipment (DME), supplies and miscellaneous items as requested by the IDT team
  • Manage and update the participant database
  • Oversee vehicle maintenance program and obtain estimates and authorize vehicle repairs per departmental policies
  • Research new vehicle options and assist in the ordering and purchasing of new vehicles
  • Ensure that vehicles have up-to-date Department of Motor Vehicle registration forms, license plates and emission stickers
  • Ensure cleanliness and proper functioning equipment as well as safe driving conditions for each vehicle
  • Ensure that all departmental staff attend and successfully execute required annual driver training courses and participate in mandatory in-service training
  • Set clear expectations and assign appropriate authority to staff; hold staff accountable for their scope of work and compliance to company policies and procedures
  • Follows all Policies and Procedures and OSHA safety guidelines
  • Maintain the confidentiality of all company procedures, and information about participants, clients, or families
  • Handle potentially infectious specimens with appropriate biohazard precautions, and practice standard precautions
  • Participate in and supports Quality Improvement initiatives
  • Comply with all state and/or federal regulations to ensure the required in-person assessments are completed in response to Service Determination Requests.
  • Comply with all state and/or federal regulations to ensure the organization is performing assessments, collecting and analyzing the data from these assessments to take actions to improve the quality of the participant’s life.
  • Comply with all state and/or federal regulations to ensure unscheduled reassessments are being conducted as required.
  • Comply with all state and/or federal regulations to ensure the IDT coordinates 24-hour care delivery that meets the needs of each participant across all care settings.
  • Comply with all state and federal regulations to ensure that all ordered, approved, or care planned services are provided to the participant.
  • Comply with all state and/or federal regulations to ensure all complaints regarding quality of care or service delivery are properly processed as a grievance.
  • Comply with all state and/or federal regulations to ensure all individual issues within a grievance are reviewed, considered, and resolved.
  • Comply with all state and/or federal regulations to ensure immediately approved SDRs have notification of the approval during the same encounter and/or interaction.
  • Participate in continuing education classes and any required staff and training meetings Maintain professional affiliations and any required certifications
  • Perform other duties as assigned

Qualifications

 

Knowledge and Critical Skills

  • Knowledge of safety requirements pertaining to the transportation industry
  • Requires individual who can coordinate multiple tasks simultaneously
  • Skilled in establishing and maintaining effective working relationships with participants, co- workers, medical staff, and the public
  • Proficient in identifying and recommending problem resolution
  • Ability to work independently with minimum supervision
  • Excellent verbal and written communication skills
  • Ability to manage a transportation scheduling computer software
  • Familiar with operation of accessible vehicles
  • Experience with maintenance and oversight of a fleet of vehicles

 

Experience

  • 2-years of administrative/supervisory experience, in a transportation setting
  • 5-years of experience in the transportation industry preferred

License / Certification

  • Valid Florida driver’s license
  • CDL License
  • Must have, or obtain upon hire, First Aid and CPR certification

Education

  • High School diploma or GED

Other Requirements

  • Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
