The Clinical Supervisor is responsible for day-to-day supervision of the nurse clinical staff and processes. Ensures quality patient care and outcomes for those enrolled in Partners In Care programs/services.
Essential Functions:
Leadership & Staff Supervision:
- Supervise field nursing staff, including evaluations and joint field visits.
- Ensure clinical competency of staff through mentoring, documentation review, and home visits.
- Provide leadership and education to staff through in-services, case conferences, and group discussions.
- Offer developmental feedback and address performance issues with staff.
- Assist in interviewing, hiring, and orienting new nursing staff in collaboration with the Clinical Educator.
- Maintain positive relationships with co-workers and staff, ensuring continuous care coordination and education.
- Facilitate care coordination by effectively communicating with staff, patients, families, physicians, vendors, and other professionals.
- Collaborate within Partners In Care programs to ensure high-quality service to patients, families, and the community.
- Maintain high standards of quality care and customer service, promoting staff satisfaction.
- Implement improvement strategies, track key operational indicators, and support organizational goals to enhance patient care and overall efficiency.
- Ensure PIC clinical programs (hospice, home health, palliative care) meet expectations by maintaining efficient workflows, monitoring customer service and quality scores, and ensuring compliance with standards.
- Assist with IDT and case conferences to ensure coordinated, quality, and cost-effective care.
- Provide input to the scheduler for patient assignments based on geography, needs, and clinician expertise.
- Review and approve RN timesheets, ensuring timely submission to payroll.
- Complete incident reports and follow-ups, using them as educational opportunities.
- Assist in policy formulation, revision, implementation, and evaluation.
- Ensure staff productivity, timely documentation, and provide financial insights to leadership.
- Adhere to Partners In Care Mission, Core Values, policies, and procedures.
- Participate in annual budget preparation and co-manage budget adjustments.
- Support program development activities and participate in committees, work groups, and special projects.
- Participate in public relations, community activities, and quality improvement initiatives.
- Perform fieldwork to assist with admissions and visits during short-staffing periods.
- Travel/driving personal vehicle throughout Central Oregon.
Required Skills & Qualifications:
- Graduate of an accredited school of nursing with current licensure to practice professional nursing in the State of Oregon.
- Bachelors degree required.
- Minimum of five years of clinical experience and 2 years of supervisory experience in the hospice or home health setting.
- Certified Hospice and Palliative Care Nurse Certification (CHPN) preferred.
- Ability to pass a background test, drug screen, and TB test.
- Valid CPR Certification.
- Current automobile insurance, valid Oregon drivers license, and reliable transportation required for those working in the field.
- Strong communication and interpersonal skills.
- Demonstrates an ability to work with other health care clinicians, development of home plan of care, knowledge of third-party reimbursement, and ability to negotiate payment rates.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Compassion, empathy, and dedication to patient care.
- This position supervises the field clinicians.
- Participates in on-call after-hours rotation for the Clinical Supervisor team.
- May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances.
- May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.
- Handles emergency or crisis situations.
- Physical demands also include the ability to lift patients, assist with ambulation, walking, carrying, and pushing.
- OSHA exposure category 1: Position includes tasks that involve exposure to blood, body fluids, tissues.
- Required Personal Protective Equipment as conditions warrant: gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap.
