Key Responsibilities

• Provide skilled, patient-centered nursing care under the direction of the supervising RN and in alignment with individualized care plans.
  
• Administer medications and treatments safely and accurately, following physician orders and agency protocols.
  
• Monitor, recognize, and report changes in patient condition with clarity and timeliness.
  
• Educate patients and caregivers on treatments, self-care, safety, and disease management.
  
• Document clinical care thoroughly and efficiently in the EMR.
  
• Collaborate with interdisciplinary partners to ensure coordinated, high-quality care.
  
• Navigate varied home environments while upholding safety, professionalism, and infection-control standards.
  

What We’re Looking For

• Current, unrestricted LVN/LPN license in the state of practice.
  
• One year of clinical experience preferred (home health experience ideal, but growth-ready candidates welcome).
  
• Strong communication skills, professionalism in the home, and confidence educating patients and caregivers.
  
• Ability to balance documentation needs with real-time patient care.
  
• Comfortable working independently while remaining a strong partner to the RN and interdisciplinary team.
  
• Current CPR/BLS certification, valid driver’s license, and reliable transportation.

Requirements:

Qualifications

• Graduate of an accredited school of practical or vocational nursing.
• Current, unrestricted LPN/LVN license in the state of practice.
• Minimum of one year of clinical nursing experience, preferably in home health, hospice, or acute care.
• Current CPR (BLS) certification required.
• Demonstrated knowledge of nursing principles, clinical techniques, and infection prevention practices.
• Proficiency in electronic medical records (EMR) documentation and basic computer applications.
• Strong communication, critical thinking, and time-management skills.
• Ability to work independently while collaborating effectively as part of the interdisciplinary team.
• Commitment to confidentiality, professionalism, and compliance with CareM policies, HIPAA, and regulatory standards.
• Valid driver’s license, automobile insurance, and reliable transportation.

Physical Requirements

• Must be able to stand, walk, bend, and assist with mobility or equipment as needed. Requires the ability to perform heavy lifting and to travel locally in various weather conditions.
