Peconic Landings long-term and short-term rehabilitative care units are recognized as one of the Best Nursing Homes by U.S. News & World Report in 2026. We are currently seeking a dedicated Per-Diem Speech-Language Pathologist to join our award-winning rehabilitative team.
We are seeking a compassionate and skilledPer Diem Speech-Language Pathologist (SLP) to provide evaluation and treatment services to residents in our skilled nursing facility. The SLP will work collaboratively with an interdisciplinary care team to address communication, cognitive, and swallowing needs, with a focus on safety, functional outcomes, and quality of life.
Job Responsibilities
- Evaluate and treat residents with speech, language, voice, cognitive-communication, and swallowing disorders
- Perform dysphagia assessments and recommend appropriate diet textures and swallowing strategies
- Develop, implement, and update individualized, evidence-based treatment plans
- Document evaluations, daily treatment notes, progress reports, and discharge summaries in compliance with facility, state, and federal regulations
- Educate residents, families, and staff on treatment plans, safety strategies, and carryover techniques
- Collaborate with nursing, dietary, rehabilitation, and medical staff as part of an interdisciplinary team
- Participate in care plan meetings and discharge planning as needed
- Maintain compliance with state practice acts, Medicare guidelines, and facility policies
Qualifications
- Masters degree in Speech-Language Pathology from an accredited program
- Current state licensure (or eligibility) as a Speech-Language Pathologist
- Certificate of Clinical Competence (CCC-SLP) preferred
- Experience in a skilled nursing or post-acute care setting preferred
- Knowledge of Medicare Part A and Part B documentation requirements preferred
- Strong clinical judgment, time management, and communication skills
