Your recruiter will provide the most up-to-date details during the hiring process._ **How you’ll make an impact in this role** + Lead and mentor a multi-disciplinary team of case management and clerical professionals to ensure seamless care transitions and high-quality disease management. + Collaborate directly with hospital administration and medical staff to optimize admission appropriateness and streamline discharge planning, directly impacting patient recovery timelines. + Empower your team as a subject matter expert by providing guidance on utilization criteria and complex case resolution, ensuring every patient receives the right care at the right time. + Optimize daily operations and staffing assignments to maintain balanced workloads, ensuring our units are always positioned to provide exceptional support to our patients and their families. **What minimum requirements you’ll need** **Licensure / Certification / Registration:** + One or more of the following required: + Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or jobtransfer date. + Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, ProfessionalCounseling & Social Work obtained prior to hire date or job transfer date. + Case Manager credentialed from the American Case Management Association (ACMA) preferred. + Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. **Education:** + High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degreerequired.
