Family Caregiver Support Program is a service available to unpaid caregivers of adults who need care in their homes. Staff provide practical information and connect the caregiver to local free or low-cost resources/services meeting their needs.
In this position, you will provide case management services to unpaid family caregivers, including information and coordination of care. Provide transformative services to individuals meeting the nursing facility level of care. Utilize a variety of assessments and screening tools to develop care plans. Seek to obtain respite and supplemental services to enrolled unpaid/family caregivers. Provide information and referral to community resources as well as long-term services and supports that best fit client needs. Facilitate caregiver support groups and caregiver classes. As appropriate, link caregivers to mental health services and other services deemed relevant and necessary. Engage in events to outreach to community partners.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Exhibit appropriate interpersonal skills and engagement strategies to complete screenings and assessments in a timely manner;
Present a variety of State caregiving programs at various public forums;
Complete timely and accurate Care Plans while ensuring the appropriate services are being offered to clients;
Request authorization for client services accurately and timely;
Facilitate caregiver support groups and educational classes;
Promote the program by participating in outreach activities;
Provide clients with information to assist in decision-making and problem-solving related to their caregiver roles;
Advocate for clients when appropriate;
Complete all documentation and entries into State databases within the required timeframe set forth by State standards;
Maintain professional acumen when interacting with clients, staff, agency partners, and leadership;
Requires travel between various FBH work sites on a regular or daily basis;
Complete all other duties as assigned by the branch supervisor and/or director.
QUALIFIED CANDIDATES WILL HAVE:
REQUIRED:
Staff administering the full TCARE® assessment/consultation and service planning must meet the minimum qualifications of an AAA Case Manager and certification in the State of Washington. These qualifications include the following minimum education and experience requirements:
A Master’s degree in behavioral or health sciences and one year of paid on-the-job social service experiences; OR
A Bachelor’s degree in behavioral or health sciences and two years of paid on-the-job social service experience; OR
A Master’s in any other field and three years of paid on-the-job social service experience; OR
A Bachelor’s degree in any other field and four years of paid on-the-job social service experience
Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire.
Must have an unrestricted and valid driver’s license, active car insurance and must be willing to use their personal vehicle in the course of work.
KNOWLEDGE, SKILLS, & ABILITIES:
Knowledge of elder care and disabled adult caregiving issues;
Ability to perform face-to-face assessments and screenings;
Proficiency with Microsoft Office suite and experience with database systems;
Experience with leading large groups and public speaking.

This position provides client services in-office and within the community at large on a occasional to frequent basis.
Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large. Some environments may not be ideal in terms of cleanliness or being habitable.
Occasionally frequent travel between locations is required.
The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.
Daily use of a computer is required.
May be required to lift 10 - 20 pounds.
Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.
PAY RANGE (Hourly/ Non-Exempt): $25.89-$31.53
All pay is based on longevity at Frontier Behavioral Health; thus the starting pay is $25.89 per hour.
