Qualifications Required:
\n Current, unrestricted Colorado Physical Therapist license
\n Graduation from an accredited Physical Therapy program
\n Valid driver’s license and reliable transportation
\n Ability to practice independently in a home-based setting
\n Proficiency or willingness to learn Homecare Homebase (HCHB)
Preferred:
\n Home health experience
\n Familiarity with Medicare documentation and home health regulations
\n Experience working with geriatric and medically complex populations
