🚨 Home Health RN Travel Assignment – Rutland, VT 🚨
\n 📍 Location: Rutland, Vermont
\n 🗓️ Contract Length: 13 Weeks
\n 🕒 Schedule: Monday–Friday, 8:00 AM–4:00 PM
\n ⏱️ Guaranteed Hours: 37 hours/week
\n 📞 On-Call & Weekends: Some required
\n 💻 EMR: McKesson
\n We are seeking an experienced Home Health Registered Nurse for a 13-week assignment. This role requires strong clinical assessment, case management, OASIS documentation, and patient education skills.
\n Key Responsibilities:
\n • Complete comprehensive patient and caregiver assessments and develop individualized plans of care.
\n • Manage assigned caseloads and coordinate care with physicians, providers, therapists, social workers, vendors, and interdisciplinary team members.
\n • Complete accurate and timely OASIS documentation, visit notes, care plans, and required agency, state, and federal forms.
\n • Provide skilled home health nursing care, including IV therapy, wound VAC management, complex wound care, infection control, and medication management.
\n • Educate patients and caregivers on medications, treatment plans, discharge instructions, and available services.
\n • Supervise and collaborate with LPNs and home care aides as needed.
\n • Support safe patient discharges, transfers, incident reporting, peer reviews, case conferences, and interdisciplinary team meetings.
\n Requirements:
\n • Minimum 3 years of Home Health RN experience
\n • OASIS experience required
\n • Active RN license or eligibility to obtain Vermont RN licensure upon acceptance
\n • Strong case management, documentation, wound care, and IV therapy skills
\n • McKesson experience preferred
\n ⏱️ Overtime: After 40 hours/week at the all-inclusive rate
