Job Description
As a Home Health LPN, you will provide skilled nursing care to patients in their homes under the direction of the Registered Nurse and physician. This role involves completing scheduled visits throughout your assigned coverage area while ensuring high-quality, patient-centered care in accordance with the plan of care and agency policies.
This position is vital in promoting patient independence, safety, and overall well-being within the home setting.
Key Responsibilities:Deliver professional nursing care within the LPN scope of practice as prescribed by the physician and directed by the Registered Nurse.
Perform skilled nursing visits including medication management, wound care, catheter care, diabetes management, patient education, and other ordered treatments.
Accurately and promptly document all clinical notes, observations, interventions, and patient progress.
Observe, assess, and report changes in patient condition to the supervising RN and Clinical Manager.
Reinforce patient and caregiver education to promote disease management and prevent hospitalizations.
Prioritize and tailor interventions based on patient needs during each visit.
Maintain compliance with all agency policies, infection control standards, OSHA guidelines, and regulatory requirements.
Participate in case conferences, staff meetings, required trainings, and agency programs.
Maintain current licensure, CPR certification, and continuing education requirements.
Comply with Medicare Conditions of Participation and all applicable state and federal regulations.
Cooperate with agency audits, documentation reviews, and quality assurance initiatives.
Graduate of an accredited School of Nursing.
Current, active LPN license in the state of practice.
Home health experience preferred.
Strong clinical assessment and observational skills.
Excellent communication and documentation skills.
Valid driver’s license with reliable, insured transportation.
Ability to travel within assigned territory.
