SUMMARY
Provides outreach or community based outpatient mental health and/or substance abuse services to individuals, groups, families and couples committed to supporting those we serve reach their individualized wellness goals. Employs a range of techniques based on experiential relationship building, dialogue, communication and behavior change that is designed to improve the mental health of the person served, or to improve group relationships (such as in a family).
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:  
Billable Functions
•Conducts psychosocial assessments to document history and identify preliminary issues for treatment by interviewing the person served (if applicable), and transcribing or dictating relevant information for person served records.
•Provides individual, couple, family and/or group therapies and family supports and crisis interventions using various treatment modalities as outlined in state/local contract and/or state licensing standards.
•Responsible for providing outreach or community based outpatient mental health and/or substance abuse services to individuals, groups, families and couples.
•Provides mental health consultation to stakeholders and treatment team members.
May or May not be billable based on the contract
•Develops individualized treatment plans in coordination with interdisciplinary team to include but may not be limited to:  The Department of Children and Families, District Family Safety/Community Based Care Provider, Juvenile Justice Counselor (if applicable), and child targeted case manager (if one is assigned)
•Works with members of the interdisciplinary team to coordinate other treatment initiatives
•Develops and coordinates discharge plan based on treatment, indicating all relevant aftercare needs and plans.  Accesses community resources that would be beneficial to person’s aftercare needs.
•Attends treatment team staffing and level of care meetings as required.
•Conducts educational groups for persons served, families and foster parents. Provides documentation educational in-services where applicable, assuring that the skills needed for treatment plan in the home environment are learned.
•Prepares and trains the family to resume care of their child (if applicable to the population being served).
•Conducts regularly scheduled face-to-face meetings with the person served and caretaker (if applicable) in order to monitor progress and discuss treatment strategies and services.  
Non-Billable Functions
•Maintains current, legible and adequate medical record documentation in chart of the person being served.  Assures that all required documentation is in their chart, including any outside agency documentation pertaining to the person served.
•Attends regular staff meetings, clinical meetings, training, and one-on-one sessions with direct supervisor to process current status of cases.
•Attend court hearings as directed by subpoena.
•Performs other related duties and activities as required.

Minimum Knowledge and Skills required by the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job: 
Education and Experience:  
•Master’s Degree in health human services field
•Experience preferred or as required by state 
oMassachusetts: Must have a Master’s degree in social work, counseling or clinical psychology or closely related field. Must have completed a full-time clinical internship in a mental health setting. License eligible preferred. Social Workers must be in process of applying for license as Certified Social Worker (LCSW).
oConnecticut: Must have a Master’s degree in social work, counseling or clinical psychology or closely related field. Must have completed a full-time clinical internship in a mental health setting. License eligible preferred
oOhio: Must hold a master's degree in social work or counseling from accredited educational institution.  Must hold a license in: Licensed Social Work (LSW), Licensed Practicing Counselor (LPC) or Marriage and Family Therapist (MFT) required
oAlabama: A master’s degree in a human service field AND 2 years of post-master’s experience in family and children’s services and professional licensure: Associate Licensed Counselor (ALC), Licensed Practicing Counselor (LPC), Licensed Masters of Social Work (LMSW), Licensed Clinical Social Worker (LCSW)
or 
A master’s degree in a human service field AND 5 years of post-master’s experience in family and children’s services
Certificates, Licenses, and Registrations:
•Licensed as required by state.
•Current driver’s license and acceptable driving record

Physical Requirements:
•Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
