The Case Manager will visit families twice a week in a pre-screened home setting to model and reinforce positive interaction between the child and parent/caregiver. The Case Manager will work with families, caregivers, and communities to support early literacy, school readiness and early opportunities. The Case Manager will be handling cases in the Bellport and/or Wyandanch communities. strongTravel is required. Must be able to drive personal vehicle to and from home visits./strong/ppnbsp;/ppstrongRESPONSIBILITIES/strong/pul type="disc"liThe Case Manager will complete a minimum of 16 hours of training delivered by the Supervisor prior to beginning home visits./liliVisit with families in the family#39;s home on a twice weekly basis, demonstrating reading, play and verbal interaction behaviors with two-year-old and three-year-old children and the parent or primary caregiver./liliThe Case Manager will complete the required paperwork on a weekly basis./liliAttend and participate in weekly supervisory meetings, staff meetings, individual supervision and training, as is required./liliAll other duties as assigned./li/ulpnbsp;/ppstrongQUALIFICATIONS/strong/ppHigh school diploma or equivalent and 5 years of experience working in human services required. Bachelorrsquo;s degree in human services or related field preferred./ppAt least 1 year of prior experience working with young children and parents in a community-based organization is required./ppMust have demonstrated knowledge of the community being served./ppProficient computer skills, including Microsoft Office, required./ppExcellent interpersonal and communication skills, both verbal and written, required./ppAbility to work independently and maintain confidentiality required./ppStrong judgement and problem-solving skills, including knowing when to seek help, required./ppBilingual in Spanish preferred./ppValid and clean NYS Driverrsquo;s License required./ppTravel is required./ppstrongnbsp;/strong/ppstrongPHYSICAL REQUIREMENTS/strong/ppMust be able to lift up to 10 lbs./ppMust be able to drive personal vehicle to and from home visits./p
