The Nurse functions as a support team member to the IDG team by providing additional services when required. The Nurse will assess, address and report patient needs to the case management team all while ensuring that all patient needs are met in a timely manner. Local travel is required./ppbr//ppstrongMajor Responsibilities/strong/pulliProvides professional nursing care by utilizing all elements of the nursing process within the designated scope of practice./liliAssesses and evaluates patient's status by:/liliWriting and initiating a plan of care (when appropriate)./liliRegularly re-evaluating patient and family/caregiver needs/liliParticipating in revising the plan of care (when appropriate)./liliCompletes all visit types requested to meet the needs of the patient and family./liliUses health assessment data to determine nursing diagnosis./liliAdministers medications and treatments as prescribed by the physician in the physician's plan of care./liliCompletes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given.
