Occupational Therapist

p style="font-family: Times;"Duties and Responsibilities:/p p style="font-family: Times;"Responsible for the assessment of referred patients that require rehabilitation services/p p style="font-family: Times;"Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation./p p style="font-family: Times;"Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician./p p style="font-family: Times;"Set realistic and achievable goals for their patients/p p style="font-family: Times;"Document and record the patientsrsquo; condition/p p style="font-family: Times;"Educate patients and families in an appropriate therapy method./p p style="font-family: Times;"Evaluations, weekly progress notes, re-certification and discharge documentations./p p style="font-family: Times;"Collaborates with all disciplines to provide best patient centered treatment and care./p p style="font-family: Times;"Perform all other duties assigned by Director of Rehabilitation/p p style="font-family: Times;"Compliance with confidentiality, HIPPA and Federal Residents Rights./p p style="font-family: Times;"nbsp;/p p style="font-family: Times;"Qualifications/Requirements:/p p style="font-family: Times;"Must have graduated from an accredited Occupational Therapist college or university./p p style="font-family: Times;"Must have a valid professional license and registration for state of practice./p p style="font-family: Times;"Experience with long-term care and/or geriatric care is preferred but not limited to./p p style="font-family: Times;"nbsp;/p p style="font-family: Times;"Benefits:/p p style="font-family: Times;"Health insurance/p p style="font-family: Times;"Dental insurance/p p style="font-family: Times;"Vision insurance/p p style="font-family: Times;"Retirement plan/p p style="font-family: Times;"Paid time off/p pnbsp;/pp/p
