This role offers an opportunity to contribute to meaningful work and help shape the future of care in communities across the country. /pp /ppOur Benefits: /pulliComprehensive medical, dental, and vision coverage/lili401(k) retirement plan/liliPaid time off and holidays/liliEmployee assistance programs and wellness initiatives/liliFlexible options to support a balanced life/liliWe offer weekly pay between $18-$20 per hour, based on experience/li/ul Responsibilities: pWhat You'll Do:/pulliMaintain elements of the assigned clients’ files, and all related paperwork./liliConsistently maintains the confidentiality of patient/client and agency information, following HIPAA guidelines relative to handling patient records./liliAssigns homecare aides based on the client’s overall needs and Plan of Care./liliProvides coaching to Caregivers to ensure high quality client care and escalates ongoing concerns with Caregiver performance to the Caregiver management team./liliProactively communicates schedules and changes with clients, home care aides, referral sources, and management./liliPrioritizes client care and service and may conduct in home visits with a client to maintain perspective and connection, assess their needs and communicate any changes or needs to appropriate parties./liliPrepares and submits routine departmental reports as required./liliRecords and maintains accurate documentation of the client’s condition and overall service./liliMaintain and keep record of client satisfaction surveys, and client in-home visits to address areas of concern as well as to continue to build relationships./liliAppropriately handles client complaints and problems; documents and reports any significant issues for further attention or resolution as required. This is a mandatory reporter position of critical incidents./liliProvides education and coaching on changes to a client’s Plan of Care./liliMaintains positive working relationships with clients, homecare aides and referral sources/liliEnsures compliance with local, state and federal laws as well as with Company policies and procedures./liliPerforms other related duties as assigned./liliIf your area of focus includes Developmental Disabilities (DD), you are required to comply with all applicable state regulations regarding training, documentation, and any other mandated practices to ensure adherence to legal and organizational standards.
