Licensed Practical Nurse

h2Our Company/h2 pResCare Community Living/p p/p h2Overview/h2 pPlans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients./p h2Responsibilities/h2 ul liProvide 1:1 care for client always following client’s plan of care while maintaining client's dignity and independence/li liMonitoring clients’ vital statistics and reporting abnormal findings to the case manager and physician assessments/li liPerforms medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results/li liComplete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist/li liEnsures infection control policies are maintained at all times, reports possible communicable diseases or infections, and ensures the overall implementation and practice of infection control procedures are implemented/li liAccurately documents nursing actions of all care given and communicates with family, case manager, and physicians as well as documents the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needs/li liOrders supplies and medications, as needed, to ensure adequate amounts are on hand at all times/li liEnsures all patient needs are being met in accordance with the instructions of the physician./li liMonitors incidents involving individuals. Ensures incident reports are completed timely, accurately, and case managers are informed; examines the individual following the incident/li liEnsures all medical equipment is maintained and reports faulty equipment to Durable Medical Equipment for repair/li liOther duties as assigned/li /ul h2Qualifications/h2 ul liCompleted post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse/li liKnowledge of individual service plans, including understanding of client rights and health needs/li liProof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice/li liCPR Certified/li liExcellent interpersonal skills, including customer service, patience, empathy, and compassion/li liIn-depth understanding of patient care policies and regulations/li liFamiliarity with symptoms and signs of improvement for several medical conditions/li liModerate amount of travel between facilities may be required /li /ul h2About our Line of Business/h2ResCare Community Living, an affiliate of BrightSpring Health Services, has five decades of experience in the disability services field, providing support to individuals who need assistance with daily living due to an intellectual, developmental, or cognitive disability.
