You will communicate with healthcare practitioners, ancillary providers, pharmacies, family members, and team members as needed ensuring timely clinical interventions and follow-up. You will lead and provide direction to Caregivers and Medication Technicians where applicable. The ideal candidate is reliable, takes pride in their work, and demonstrates empathy.
