divCMHA is looking to hire a Full-time, 2nd shift Case Manager for our Transitional Living Center (TLC) program. This position will be located at 66 Clinic Dr. New Britain, CT 06051. Sunday - Thursday, 3:30PM - 11:30PM. /div

divbPosition Summary/b: The Case Manager will be responsible for engaging and linking current or potential homeless clients with mental health or co-occurring disorders to MAT services. Serves as a liaison between CMHA and identified community providers to coordinate various aspects of care, including clinical services, housing support and individualized case management, as needed, to complement current services. Assist clients to ensure stable community living. /div

liProvides advocacy, support and guidance with contacting external organizations (Social Security, DSS, DCF, DMHAS, etc)./li
liEducates clients to utilize emergency and community resources to help meet basic needs./li
liWorks with clients to teach life skills necessary to live independently in the community. This includes assisting with finances and entitlements, supporting the development of activities of daily living, nutrition, housing maintenance and repair, and prompting to encourage task completion./li
liCollaborates and integrates with other CMHA teams to ensure all services provided to client are communicated with clients whole treatment team/li
liAssures timely documentation is maintained in accordance with CMHA policies and procedures, including but not limited to progress notes, treatment plans, authorizations, program orientation, and consent to treat, activity logs, and monthly targeted case management notes. Maintains appropriate caseload as determined by CMHA and funder/li
liPerforms other related duties as assigned./li

liBachelor’s degree in a mental health related field preferred or equivalent experience/li
liMust have valid driver’s license/li
liWill become certified in MANDT, First Aid, and CPR./li
liParticipate in training courses on an annual basis, as assigned./li

liBeginner to intermediate experience with Microsoft Office products including Word and Outlook./li
