This position will be located at 66 Clinic Dr. New Britain, CT 06051. Sunday - Thursday, 3:30PM - 11:30PM. /div div /div divbCompensation Range:/b The salary range for this position starts at $20 an hour. /div diviCompensation for each position is determined not only by years of relevant experience, education, and skills, but also by maintaining internal pay equity. CMHA also takes into consideration our benefits and paid time off package to provide a well-rounded and competitive approach to the applicant’s overall compensation./i/div div /div divbPosition Summary/b: The Case Manager will be responsible for engaging and linking current or potential homeless clients with mental health or co-occurring disorders to MAT services.  Serves as a liaison between CMHA and identified community providers to coordinate various aspects of care, including clinical services, housing support and individualized case management, as needed, to complement current services.  Assist clients to ensure stable community living. /div ul liProvides advocacy, support and guidance with contacting external organizations (Social Security, DSS, DCF, DMHAS, etc)./li liEducates clients to utilize emergency and community resources to help meet basic needs./li liWorks with clients to teach life skills necessary to live independently in the community. This includes assisting with finances and entitlements, supporting the development of activities of daily living, nutrition, housing maintenance and repair, and prompting to encourage task completion./li liCollaborates and integrates with other CMHA teams to ensure all services provided to client are communicated with clients whole treatment team/li liAssures timely documentation is maintained in accordance with CMHA policies and procedures, including but not limited to progress notes, treatment plans, authorizations, program orientation, and consent to treat, activity logs, and monthly targeted case management notes.
