Our employees make all the difference in our success!/b/pp style="text-align:inherit"/pp style="text-align:inherit"/pUnder RN supervision, the LPN, Continuous Care is responsible for providing nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.p style="text-align:inherit"/pp style="text-align:inherit"/ppQualifications:/pulliA high school diploma or G.E.D. or an equivalent combination of education and experience/liliCurrent LPN licensure in the state of employment/lili6 months experience as LPN preferred/liliActive BLS for healthcare professionals from the American Heart Association or Red Cross./liliFor current Hope Employees: BLS should be obtained by January 1, 2025/liliFor current Capital Caring employees: BLS should be obtained by November 1, 2024/liliMobile Driver - Valid driver’s license and automobile insurance per Company policy/liliReliable transportation to meet visit schedule/liliIf working in an inpatient unit certificate of completion of 30 hour Infusion Therapy Course and in-house 8-hour Infusion Class preferred/liliAbility to communicate (written and verbal) in English/liliFlexible schedule to meet the needs of patients/liliAbility to use equipment with visual and auditory mechanisms/liliAbility to manage the emotional stress of working with and caring for terminally ill patients and their families/liliAbility to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation/liliCan work within their scope or practice per state regulations/li/ulp/ppCompetencies:/pulliSatisfactorily complete competency requirements for this position./li/ulp/ppResponsibilities of all employees:/pulliRepresent the Company professionally at all times through care delivered and/or services provided to all clients./liliComply with all State, federal and local government regulations, maintaining a strong position against fraud and abuse./liliComply with Company policies, procedures and standard practices./liliObserve the Company’s health, safety and security practices./liliMaintain the confidentiality of patients, families, colleagues and other sensitive situations within the Company./liliUse resources in a fiscally responsible manner./liliPromote the Company through participation in community and professional organizations./liliParticipate proactively in improving performance at the organizational, departmental and individual levels./liliImprove own professional knowledge and skill level./liliAdvance electronic media skills./liliSupport Company research and educational activities./liliShare expertise with co-workers both formally and informally./liliParticipate in Quality Assessment and Performance Improvement activities as appropriate for the position./li/ulpJob Responsibilities:/pulliUnder the direction of an RN, provides nursing tasks to patients and families such as continuous care, dressing changes, catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG)./liliDemonstrates clinical skills regarding application of Company policies and procedures such as:  medication administration, symptom management, and patient/family relationships./liliObserves/monitors patients for alterations in body systems, pain and symptoms and reports to RN./liliObserves and reports signs of adverse reactions and responses to treatments and reports to RN./liliReports findings to physicians, APRNs and physician’s assistants following collaboration with RN./liliPerforms venipuncture for blood specimens./liliPerforms Central Venous Line (CVL) site observation/inspection;  venous access device dressing, cap change and flushing venous access device; IV medication and fluid administration, IV pushes (per procedure) and venipuncture.  May not make any changes to infusion pump including, but not limited to, rate changes and IV bag changes./liliDocuments per policy and procedure in medical record./liliDuring times of emergencies (i.e. Hurricanes, etc.), the LPN may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency/liliPerforms other duties as assigned./li/ulpSome locations may require:/pulliCross training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing./li/ulp/pp/ppbCompensation Pay Range: /b/p$21.64 - $31.17p/pp/ppbThis position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy./b/pp/ppspanbAll Chapters Health System employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment is made as well as ongoing eligibility.
