This role offers an opportunity to contribute to meaningful work and help shape the future of care in communities across the country. /pp style="margin: 0px;" /pp style="margin: 0px;"Our Benefits: /pulliComprehensive medical, dental, and vision coverage/lili401(k) retirement plan/liliPaid time off and holidays/liliEmployee assistance programs and wellness initiatives/liliFlexible options to support a balanced life/liliWe offer weekly pay between $18.00-$21.00 an hour, based on experience/li/ul Responsibilities: p style="margin: 0px;"What You'll Do:/pulliMay maintain elements of the assigned clients’ files, employee files and all related paperwork./liliConsistently maintains the confidentiality of patient/client and agency information, following HIPAA guidelines relative to handling patient records./liliMay provide direct training./liliAssigns homecare aides based on the client’s overall needs and Plan of Care./liliMonitors the performance of assigned homecare aides who provide and support patients with personal care to ensure quality service./liliProactively communicates schedules and changes with clients, home care aides, referral sources, team leads, managers and the branch manager./liliMay conduct in home visits with a client per month to maintain perspective and connections to the Client and Caregiver experience./liliPrepares and submits routine departmental reports as required./liliRecords and maintains accurate documentation of the client’s condition and overall service./liliMay, where applicable, conduct and keep record of client satisfaction surveys, client in-home visits, employee evaluations and employee in-service trainings./liliAppropriately handles client complaints and problems; documents and reports any significant issues for further attention or resolution as required.
