The LPN assists with patient care planning, implementation, and evaluation while ensuring a safe and compassionate environment for patients and families/font/span/p/font/ppfont face="Arial"/font/pp /ppbfont face="Arial"Responsibilities/font/b/ppfont face="Arial"pspan style="font-size:11pt"· /spanspan style="font-size:11pt"Provide direct patient care, including monitoring vital signs, administering medications within scope of practice, and assisting with daily activities./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Collaborate with RNs and physicians to develop and implement patient care plans./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Document patient care activities accurately in electronic health records (EHR)./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Perform wound care, catheter care, and other clinical procedures as required./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Assist patients with mobility, hygiene, and nutrition as needed./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Educate patients and families about health conditions, medications, and post-hospital care./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Maintain a clean, safe, and supportive environment for patients./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Adhere to hospital policies, infection control procedures, and safety protocols./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Respond promptly to patient calls and address concerns with compassion and professionalism./span /ppspan style="font-size:11pt"· /spanspan style="font-size:11pt"Participate in ongoing training and professional development programs./span /pul/ul/font/ppfont face="Arial"/font/pp /ppbfont face="Arial"Knowledge, Skills amp; Abilities/font/b/ppfont face="Arial"Patient Group Knowledge (Only applies to positions with direct patient contact)br /br /The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department’s identified patient ages.
