Work where you matter./b/pp style="text-align:inherit"/pbrbA Brief Overview/bbrbrEvaluates patients, develops treatment plans and provides treatment for medically necessary speech therapy services addressing a patient's deficits in speech, language, cognition, voice and dysphagia in the home setting. The Speech Pathologists demonstrates the knowledge and skills necessary to provide care for the population served.brbrbWhat you will dobr/bbrulliEvaluates assigned patients through selection, administration, and interpretation of formal and informal diagnostic assessments to determine the levels of functioning and intervention needs in all areas of speech pathology./li liDemonstrates knowledge of age-specific factors and performs satisfactorily related to patient’s age infant through geriatrics as noted on unit specific guidelines and competencies./li liDevelops and implements individualized treatment programs for the patients needing services./li liEstablishes and modifies intervention goals and strategies for patients/ treatment program based on on-going evaluations. Uses selected activities and/or tasks to promote and maintain health, and to achieve treatment goals./li liProvides direct therapy services to patients using various functional settings and strategies./li liPlans therapy with in the patient’s individual physical, emotional and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans./li liPrepares written documentation as required by the profession and the department, such as evaluation results, individualized treatment plans, progress notes, and other such reports./li liCommunicates effectively with patient, family, physician, and other disciplinary team members regarding the patient status, progress, and needs./li liEstablishes rapport and work cooperatively with interdisciplinary team members (including family/physician) throughout the patient’s intervention program./li liParticipates in program development of the department.
