LPN (Med Pass) (3:30pm to 7:30pm)

Our mission is to provide the highest level of care for each of our residents. The ideal candidate will be experienced in performing a variety of care practices under the direction of the Registered Nurse and/or Unit Supervisor./p pstrongResponsibilities:/strong/p p- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs./p p- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans./p p- Document patient information, medical history, and treatment plans accurately and thoroughly./p p- Assist with case management and coordination of care for patients with complex medical needs./p p- Conduct physical examinations and assessments to determine patient health status./p p- Provide education and support to patients and their families regarding healthcare management./p p- Administer basic life support and emergency care as needed./p pstrongExperience:/strong/p p- Must have a valid LPN license in the state of NY./p p- Experience working in a hospital or healthcare setting preferred./p p- Proficiency in using electronic medical record systems, such as Athenahealth./p p- Knowledge of medical coding and documentation practices./p p- Familiarity with occupational health procedures and protocols./p p- Ability to suture wounds and perform basic wound care./p p- Experience working with individuals with developmental disabilities is a plus./p pnbsp;/p pstrongJob Type: /strongFull-time/p pnbsp;/p pPhysical setting:/p ul class="editor-ul" li pLong term care/p /li li pRehabilitation center/p /li /ul pStandard shift:/p ul class="editor-ul" li pDay shift (3:30pm to 7:30pm)/p /li /ul pWeekly schedule:/p ul class="editor-ul" li pRotating weekends/p /li /ul pAbility to Relocate:/p ul class="editor-ul" li pWest Babylon, NY 11704: Relocate before starting work (Required)/p /li /ul
