Place yourself in an environment where your growth is supported, your contributions valued, and your successes celebrated./p ul listrongFocus on CARES values/strong: Commitment, Action, Respect, Excellence, and Service/li listrongFocus on Employee Wellness/strong: Biometric screenings, Wellness programs, Access to onsite gym, Zen Room, Community Shared Agriculture program/li listrongStaff Recognition platforms/strong: Shining Stars, CARESCount website /li /ul p /p pstrongMain Functions:/strong/p ul liWorks collaboratively with a team of Clinical Dietitians to provide comprehensive nutrition care in the long-term care setting./li liDevelops and implements individualized nutritional care plans for residents./li liDocuments all aspects of nutrition care using standardized documentation./li liDemonstrates the CARES values at all times./li /ul pstrongResponsibilities:/strong/p ul liDemonstrates the ability to work independently and exhibits flexibility in decision making and prioritizing/li liDevelops and implements individual care plans for long-term care residents/li liProvides on-going nutritional assessments, monitoring, evaluating and education to residents and families/li liIntegrates evidenced based research in the education, practice and management of residents/li liResponsible for timely and pertinent documentation of long-term care avenues as directed by department policies and procedures, professional judgment, provider orders and staff recommendations/li liDemonstrates accountability for the proper use of residents’ protected health information and adherence to institutional safety practices/li liActively guards the confidentiality of sensitive information including but not limited to that of residents, staff, and the health system/li liCompliant with all regulatory requirements included but not limited to requirements by JCAHO, DOH, HIPPA, and OSHA/li liMaintains expertise by pursing pertinent continuing education opportunities/li liAddresses questions and concerns of residents and families and health professionals in accordance with evidence-based nutrition practice and critical thinking and assessment skills/li liEffectively communicates nutrition plans and rationale to team members both verbally and in the medical record/li liParticipation in avenue specific, disease specific interdisciplinary rounds/li liServes as a liaison between residents, avenues and food service staff/li /ul pstrongQualifications: /strong/p ul ul liBachelor’s degree required in accredited nutrition dietetics or related field/li liCompletion of Dietetic Internship or ASCEND approved Graduate Program/li liRegistery eligibility required/li liRegistered Dietitian and CD-N preferred/li liOne year in health care setting as a Dietitian or Diet Tech preferred/li /ul /ul p /p pstrongPay Range: /strong$28.00 - $35.00/p pstrongStarting Pay: /strongBased on Experience/p pstrongemThompson Health is an EOE encouraging women, minorities, individuals with disabilities and veterans to apply/em/strong/p p /p
