This role will be responsible for conducting comprehensive assessments and facilitating group and individual sessions for patients seeking assistance with substance use disorders./ppbr//ppAs the Outpatient Counselor you will:/pulliConduct comprehensive substance use evaluations for new patients to determine their individual needs and the level of care required./liliCollaborate with community partners to conduct evaluations in person and via telehealth for incarcerated patients./liliComplete engagement activities and partner with Supervisor to ensure patients exiting incarceration are assigned a regular Counselor upon release./liliLead group counseling sessions for patients; providing education, support, and guidance in a supportive group setting./liliCollaborate with the clinical team on patients’ individualized treatment plans based on assessment findings, outlining specific goals and objectives./liliMaintain timely, accurate and confidential records of patient sessions, including assessments, treatment plans, progress notes, and group attendance records, in compliance with agency policies and regulations./liliProvide urinalysis monitoring on an as needed basis./li/ulpbr//ppbr//ppRequirements/ppQualified candidates will have a bachelors degree in a human services field and one year of related work experience. This position requires a valid driver's license and the ability to pass an extensive background check/ppbr//ppASAC offers competitive compensation, this role starts at $24.07 with additional compensation for candidates with extensive experience.
