The PA has responsibility, authority and accountability for the provision of care through collaboration with a physician. The APP manages and provides patient care activities for a group of patients and their families through the application of independent judgment, communication and collaboration with all team members. The APP establishes and maintains collaborative relationships with physicians, other health care providers, patients and their families, to achieve desired patient outcomes throughout the continuum of care. The APP shall work in collaboration with a physician./font/pp /ppspan style="font-weight:800"font face="Arial"Responsibilities/font/span/ppfont face="Arial"/font/ppfont face="Arial"/font/ppfont face="Arial"Collaboration/font/ppfont face="Arial"/font/ppfont face="Arial"Interacts with various members of the health care team to meet patient care needs and collaborates with other providers to identify changing patient needs. Identifies teaching/learning needs of patients, documents patient care services in the electronic health record./font/ppfont face="Arial"/font/ppfont face="Arial"Communication/font/ppfont face="Arial"/font/ppfont face="Arial"Exhibits skill in the management of conflict situations, functions so as to promote trusting relationships and demonstrates open and honest communication at the unit level. Participates in performance improvement activities as appropriate and promotes and fosters an atmosphere of teamwork and excellence among staff./font/ppfont face="Arial"/font/ppfont face="Arial"Job Skills/font/ppfont face="Arial"/font/ppfont face="Arial"Conducts physical examinations, initiates testing and treatment based on scope of practice, identifies normal and abnormal findings and acts upon abnormal findings accordingly. Refers patients to appropriate specialists and/or allied health professionals and provides counseling and instruction regarding common patient problems./font/ppfont face="Arial"/font/ppfont face="Arial"Safety/font/ppfont face="Arial"/font/ppfont face="Arial"Protects patients, visitors and employees by adhering to infection-control policies and protocols, and identifying patients using two identifiers (i.e., name and date of birth) prior to providing any treatment or procedure.
