We are proud of our mission, our lasting community impact, and the cultural and individual diversity of our staff./span/pp/ppbJob Summary/b/ppUnder supervision of the Manager, Health Enrollment the Coordinator of Access to Care - GRANT position facilitates the day-to-day activities of FHCSD#39;s health insurance screening and application services./pdivpbr /bJob Descriptionbr /Job Roles/b /pulliCoordinates communications across departments, notifying and advising other departments and supervisors./liliCoordinates community events and coverage. Conducts health insurance education at appropriate clinic and community sites. Attends relevant Health Coverage trainings and local coordination meetings./liliCoordinates the day-to-day activities of the department, including staff scheduling, training for new staff, supporting team to meet grant goals, overseeing daily work, and ensuring staff productivity goals are met./liliDocuments contacts; maintains files; and submits program statistics as needed. Implements and maintains an enrollment verification system to document outcomes.-/liliProvides health insurance eligibility determination and application assistance as assigned clinic and community sites./liliProvides monthly reports on engagement and application assistance activities./liliPerforms other duties as assigned/li/ulp/ppbEducation/Certifications/Licenses/Registrations /b/pulliHigh School Diploma or GED Equivalency required./liliSome college coursework preferred./liliTraveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency. Mileage and other reimbursement governed by policy./li/ulp/ppbExperience/Specialized skills (including Language) /b/pulli1 year experience coordinating groups or projects required./lili2 years of supervisory or lead experience preferred./lili3 years of experience in a community-based health or social service agency organization preferred./liliAbility to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high-workload environment./liliAbility to work well in both a team-based environment and independently./liliBasic knowledge of Medi-Cal, CalFresh, and private insurance./liliBasic knowledge of medical terminology./liliBilingual in English/Spanish required; speaking, reading and writing skills in both languages./liliWritten and verbal communication skills, including ability to communicate effectively with patients and family members who speak English as a second language./liliDemonstrated ability to be culturally sensitive and respect diversity./liliAbility to work effectively with patients from diverse social, cultural and economic groups./liliBasic knowledge of Medi-Cal, CalFresh, and private insurance./liliKnowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners./liliBasic computer literacy ability to comply with department needs and expectations (i.e., Microsoft Excel and Word)./li/ulp/p/divp style="text-align:inherit"/pp style="text-align:left"In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits./p21.00 - 28.1025p style="text-align:left"If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
